Decree of March 3, 2025, amending the decree of October 16, 2017, setting out the template for fitness and unfitness notices, individual health monitoring certificates, and proposals for workplace adjustments.

The new templates for fitness notices, unfitness notices, individual health monitoring certificates, and proposals for workplace adjustments have been set by a decree dated March 3, 2025, published in the Official Journal on March 15, 2025. The implementation date for these new templates is set for July 1, 2025. These updated versions incorporate the changes made by Law No. 2021-1018 of August 2, 2021.

This decree includes four annexes, each corresponding to a document that an occupational doctor or another authorized healthcare professional from the Occupational Health and Safety Service (SPST) may provide during an employee’s medical examination:

  • Annex 1 of the decree: The individual health monitoring certificate, issued at the end of all medical visits (excluding pre-return-to-work visits).
  • Annex 2 of the decree: The fitness notice reserved for workers under enhanced individual health monitoring.
  • Annex 3 of the decree: The unfitness notice, which the occupational doctor may issue at the end of any medical visit (excluding pre-return-to-work visits).
  • Annex 4 of the decree: The document recommending workplace adjustments, adaptations, or transformations, or adjustments to working hours, which may be issued along with the health monitoring certificate or the fitness notice.

The main changes introduced by the decree of March 3, 2025, are as follows:

  • The individual health monitoring certificate no longer includes a signature field for the employee.
  • The health monitoring certificate now includes the new types of health examinations provided for by Law No. 2021-1018 of August 2, 2021, known as the “Occupational Health Law” (post-exposure, post-professional, and mid-career medical visits).
  • A note has been added to the unfitness notice to highlight the exceptional nature of cases where the obligation to reclassify the employee is waived.
  • The templates now include a field for the INS DataMatrix (National Health Identifier).
  • These documents must now include the employee’s INS registration number, place of birth, and Insee code.
